Welcome to Rebel Rising Wellness's client consultation scheduling page! This is a complementary 15 minute video session to determine if I may be able to help you with your specific concerns and if we may be a good fit to work together.

 

Once you have requested an appointment, you will be emailed a confirmation. Once the appointment request is approved, you will be emailed a link to join at the time of your call.

Please Note: I will accept Medical Mutual until 1/1/2025. I am an out of network provider with all other insurance companies. My fee is $175 for the initial session and $160 for each individual psychotherapy session.

For those clients who choose to bill their insurance providers, per their out of network benefits, I will provide a Superbill which contains all of the clinical information that is required by them to process a claim.

Note also, that a mental health diagnosis is required by all insurance companies to process your claims.
